Identify Who is in the Headcount

The Field Trip Request Form

We will need to know the following:

1. The names of the teachers who are bringing their class.

2. The number of students each teacher has in his/her class.

3. Do any students have special needs?

4. The number of chaperones you intend to bring with your group.

Download a Request Form, complete it, and fax to 337-291-8061 or email to Scheduling Desk.

Call the scheduling desk at 337-291-5544 and the scheduling coordinator will ask you questions and complete a request form for you.

You will receive a confirmation via mail, email, or fax depending on the method requested on the form.
